Marionette Lines Filler
Marionette lines are the lines that run from the corners of the mouth down towards the chin, often contributing to a tired or downturned appearance.
Dermal filler is used to carefully soften these lines by restoring lost volume and supporting the surrounding structures of the lower face.
Treatment is tailored to your individual facial anatomy, with a focus on maintaining natural movement and expression.
In some cases, optimal results are achieved by treating surrounding areas such as the chin or cheeks, as part of a full-face approach.
Softens marionette lines
Reduces a downturned or tired appearance
Restores volume and support to the lower face
Improves facial balance and proportions
Maintains natural facial expression
Immediate, visible results
Please attend your appointment well hydrated and having eaten beforehand. Do not wear makeup on the day of your appointment.
Your lower face will be assessed and a personalised treatment plan will be discussed, including whether treatment of adjacent areas may be recommended.
The skin will be cleansed and prepared. A topical anaesthetic may be applied if required.
Dermal filler is then carefully injected into the marionette lines and surrounding areas using precise techniques to achieve a natural, balanced result.
The procedure typically takes around 30–45 minutes.
Mild swelling, tenderness, or bruising may occur and typically settles within a few days.
Avoid makeup for 24 hours
Avoid strenuous exercise, saunas, and heat exposure for 24–48 hours
Avoid touching or massaging the treated area unless advised
Avoid alcohol for 24 hours
Use SPF daily
Final results are best assessed once any swelling has settled, usually within 1–2 weeks.

How long do the results of marionette line filler last?
The results for the lower face typically last between 9 to 15 months. Because the area around the mouth is highly dynamic and subject to constant movement from speaking, smiling, and chewing, the hyaluronic acid naturally metabolizes slightly faster than in static structural areas like the cheeks or chin. To maintain optimal support and keep the lower face looking softly lifted, we generally recommend scheduling a clinical review around the 9 to 12-month mark.
Does the marionette line filler procedure hurt?
The treatment is highly tolerable and designed with your absolute comfort in mind. We apply a premium topical anesthetic cream to the area around your mouth prior to the procedure. Additionally, the advanced dermal fillers we use at Zorah Clinic are formulated with lidocaine, a built-in local anesthetic that numbs the deeper tissues from the inside as the gel is carefully placed. You may feel a mild, dull pressure or pushing sensation, but it is not typically considered painful.
Will filler in my lower face make me look heavy or unnatural?
Absolutely not. A common fear is that treating the lower face will result in a bulky or artificial appearance. At Zorah Clinic, our primary goal is elegant restoration. Deep marionette folds are often the result of volume loss higher up in the mid-face combined with descending tissue. We use highly advanced, conservative techniques to place the filler precisely where structural support is needed, gently lifting the corners of the mouth and smoothing the shadowy fold without adding any heavy, undesirable width to your lower face.
What is the downtime, and will I be noticeably swollen?
It is completely normal to experience mild swelling, localized redness, and tenderness around the mouth immediately following the procedure. Because the lower face is highly vascular and contains many small blood vessels, minor bruising is also a common and expected biological response. The initial swelling typically subsides within 3 to 5 days, and the dermal filler will beautifully soften and integrate with your natural tissue over the following 2 weeks.
Can marionette lines be treated with anti-wrinkle injections instead of filler?
While dermal fillers are exceptional at restoring lost volume and physically smoothing deep, static folds, they are not always the only required solution. Often, a heavy, downturned smile is exacerbated by overactive depressor muscles (the DAO muscles) pulling the corners of the mouth downward. During your consultation, your clinician will meticulously assess your facial dynamics and may recommend a highly effective combination approach: using precise anti-wrinkle injections to relax the downward muscle pull, while using dermal filler to beautifully restore the underlying structural volume.
